Stork Foundation Grants
As a charitable organization, we are limited by the amount of funds that are donated and therefore cannot offer grants to all who apply in a given year. Grants up to $15,000 per family are awarded on an annual basis. Funds are paid directly to the provider on behalf of the patient. Our annual grant cycle runs August 1-September 30.

GRANT ELIGIBILITY REQUIREMENTS ARE LISTED BELOW:
- 18 years of age or older.
- Annual household income of $150,000 or below.
- Medical documentation required: A diagnosis of infertility must be provided by a Board Certified Reproductive. Endocrinologist or documentation outlining your necessity for fertility treatment.
- Applicant must have little or no infertility insurance coverage, or have exhausted infertility coverage benefits.
- Reside in the United States.
- Recipients must begin treatment within six months of receiving a grant.
- Complete the application in its entirety
- Write a brief (1,000 words or less) story that highlights your fertility journey
- Have a doctor complete and submit the required medical form
In addition to meeting the above requirements, applicants must also demonstrate overall good physical and mental health, and show financial stability including basic health insurance for mother/prenatal care, adequate living arrangements and means for child support.
**Please Note: A clear background check and proof of income through recent W2 verification will be required for all final grant recipients. Grants will not be awarded for those who do not meet eligibility criteria as outlined above.

Do both partners need to be residents in the US?
Yes, both partners need to be legal residents of the US.
Do I need to have an infertility diagnosis to be eligible for a grant?
Yes, you do need a formal diagnosis of infertility from your doctor to be eligible to receive a grant. The exceptions to this are for same-sex couples, applicants who are single, or recently diagnosed with cancer.
Are grants awarded only to couples?
The Stork Foundation for Infertility grant program is open to all individuals over the age of 18 regardless of gender, marital status, or sexual orientation.
What is the maximum annual income to be able eligible for a grant?
Applicants must have an annual household income of $150,000 or less to be eligible to receive a grant.
Is there a timeline for using grant funds?
Yes, you must start treatment within 6 months of receiving grant funding. Funds must be used within 1 year of the treatment start date. Funds not used in this time period will be forfeited.
Can grants be used to cover cycles or procedures already completed?
No, grants cannot be used to reimburse past cycles or medical procedures. Grant funds can be used for any treatment starting the day the grant has been awarded.
What treatment or services are covered by a grant?
Grants are paid directly to the clinic or service provider and can be used to cover the costs of IVF, IUI, cryopreservation, cycle medications, gestational carrier, embryo testing, and donor embryo, egg, or sperm. Grants are awarded up to $15,000. Applicants are responsible for costs exceeding the amount of their grant.
Is this grant for individuals who currently have fertility coverage on their insurance?
Yes, our grants are open to individuals or couples who have no coverage or limited coverage through their insurance plan. Recipients may be asked to provide up-to-date insurance coverage details upon receiving their grant award.
For grant recipients with insurance benefits, the cost of treatment will need to be billed to insurance first and the grant will cover the remaining cost, up to the grant award total. If it is determined at the time the grant is being paid that a grant recipient has majority coverage insurance, the recipient will be considered no longer qualified and will forfeit their grant funds.
Can I submit a paper application?

No, our goal is to eliminate as many financial barriers for our potential grant recipients and the application process is free.
How many grants are awarded each grant cycle?
The number of grants awarded is entirely dependent on fundraising. Not everyone who applies will be awarded a grant.
Can I apply for a grant more than once?
Yes, applicants may apply more than once. You will need to submit an updated medical form and story along with your application for each new grant application cycle. Individuals and/or couples can receive a lifetime grant maximum of $15,000.
When will I hear back about the status of my application?
All applications are reviewed in October and the status of your application will be updated via email by mid-November.
